Step 1: Visit the website of Passport Seva Kendra (PSK), log in or create an account
Step 2: Click on “Reissue of Passport” and choose “Change in Existing Personal Particulars”. In this portal, you must add spouse name in passport.
Step 3: Fill in all details and add documents. You must add your marriage certificate if you have one. If you do not, you must go for “Annexure J”.
Step 4: After that, you need to book an appointment at your nearest PSK office to verify all submitted documents.

Step 6: Visit the PSK Office as scheduled and verify documents with the payment receipt.
Step 7: Wait and track your application status, and within a few weeks you must receive an updated passport.

The documents for adding spouse name to your passport are,
